Secondly, review the Accepted Insurance and Fee page to ensure that we accept your health insurance carrier. In addition, you need to understand the Self-Pay, Co-payment (if required), Cancellation Fee and other Contractual Service Fees, which may vary in price/rate. Next, you will be required to complete every document under the eForm Portal, except for the POWER LLC Welcome Letter. The POWER LLC Welcome Letter is for your reference and record only. However, it has valuable information for each new client to refer to in the event of a crisis situation. Take your time to complete all other eForms by reading and answering all pertinent questions or information in its entirety. Please be as detailed as possible when completing all your eForms, especially the New Client Intake Form, which is a total of two pages. Finally, it is vital that you attach or upload a front and back image of your state-issued Driver’s License or identification, a front and back image of your health insurance card, and a front image of your social security card. If you complete the Pre-Client Intake Process just as we have explained for you to do so, you will be scheduled for your first Teletherapy visit, which consists of the Initial Evaluation, expeditiously.

 POWER LLC is required to obtain the aforementioned required documents from all New Clients because of the thorough verification process that is completed to ensure the safety of POWER Private Practice LLC and each New Client. New Clients must submit all three (3) documents due to the drastic increase in fraud and scamming. POWER LLC verifies all documentation by way of a secure, trusted portal, where our biller ensures that all clients are eligible, his/her health insurance is active, and we also inform clients when he/she needs to re-certify for health insurance coverage so that there is no disruption in services. POWER LLC wants all of its potential clients to feel welcome and comfortable, so you can also print out all of the eForms, you can fill it out manually by writing neat and legible in blue or black ink, and scan all ten (10) physical forms, which will be a total of 14-pages not including the Welcome Letter. In addition, you will upload or attach the three (3) required documents (i.e. drivers license/ID, health insurance card, and social security card) and email everything to the following email address: powerprivatepracticellc@gmail.com . Please be mindful that a few of the eForms may have 2-pages, so you will need to scan all paperwork in its entirety. When emailing your New Client Intake paperwork, please write your first and last name in the subject line. For example, (RE: John Doe) or you can feel free to upload all paperwork and required documents (drivers license/ID, health insurance card, and social security card) to the website, whichever is easier.

As a New Client, if you complete the Pre-Client Intake Process exactly how POWER LLC has instructed you to do so in the previous questions from start to finish, your paperwork will be processed within three (3) business days. Generally, your first Teletherapy session will be scheduled within five (5) business days, where the day of the clients’ appointment, all New Clients will need to have pertinent information such as his/her medication list (i.e. the name of relevant medication, especially psychotropic medicine and/or Medication Assistance Treatment (MAT), and dosage/frequency), the contact information of all prescribers, meaning clinicians who prescribe your medication, the name of any previous Psychiatrist, Mental Health Therapist and Primary Care Physician. This information will be useful when completing your Initial Evaluation. After a New Client completes his/her Initial Evaluation, the second scheduled session, which is normally the second week; he/she will work closely with the clinician to establish a solid therapeutic treatment plan. The purpose of a holistic treatment plan is to guide each New Client independent therapeutic journey and healing process, while being in active therapy and working towards accomplishing achievable goals.

Generally, POWER LLC Psychotherapist starts each New Client with an Initial Evaluation during the first tele-health visit.  Based on the New Clients’ Initial Evaluation, the clinician usually meets clients on a weekly basis based on the severity of his/her diagnosis.  Once a client starts to meet his/her therapeutic goals and exhibit progress, the clinician will meet with the client on a biweekly basis.  Lastly, if a client continues to display moderate to good progress, then clinician continues to monitor and follow-up with client on a monthly basis.  Once you are an active client under POWER Private Practice LLC, if you are content with our premier services, you are able to be a client for approximately three years.  At any time or for any reason, a client can communicate, if he/she would like to be discharged orally or in writing.  Lastly, if a client has a tele-therapy visit, he/she must cancel 24-hours prior to the scheduled visit or client will be responsible for a No Show/Cancellation Fee of $105.  For instance, you have a scheduled appointment on Thursday at 1:00 pm, client will need to cancel by Wednesday at 1:00 pm and is responsible for rescheduling his/her next tele-health session when time permits.
